Your first visit to our office is designed to get you acquainted with all that we offer, as well as introduce you to Dr. Goei and our caring dental family. We encourage questions and do our best to always deliver the quality care you and your family deserve.

Prior to scheduling your appointment, please let us know if you have any of the following conditions that require pre-medication: heart murmur, knee or hip replacement, cosmetic surgery or let us know if you‘ve previously been told you are required to be pre-medicated before dental visits. If we will be working with your dental insurance, please bring your card. If your insurance does not provide you with a card, please bring the company name, phone number, and your member ID number to help us in the verification of your dental benefits.

For your convenience purposes, we have now added our new patient forms to our website.  This gives us more time to spend with you on your very first visit.  Please download, fill them out and print prior to your appointment.  If you have any problems downloading the files properly, or do not have a printer please call us at either location and we would be more than happy to fax or mail them to you.
